(1)講座・分野の紹介

整形外科は骨折や外傷の治療だけではなく、脊椎や関節など運動器に関する分野です。
このため、整形外科の臨床研究ならびに基礎研究においては骨、軟骨、関節、靭帯、腱、神経など様々な組織が含まれます。
平成29年4月に整形外科学講座に齋藤貴徳教授が就任されました。これからより一層基礎研究に力をいれていく方針です。基礎研究は身近なテーマから起こる疑問を突き詰めていくことが第一歩です。
例えば、骨折後の骨癒合や骨リモデリングを促進させるためには骨芽細胞と破骨細胞の骨代謝を研究する必要があります。また、歩行をスムーズに行うためにはキャプチャーモーションと筋電図を組み合わせて関節の動きと筋肉の活動を調べる必要があります。
さらに、臨床医ならではの取り組みもあります。臨床医にとって術後感染は頭が痛い合併症の一つです。
この合併症の初期の段階で感染起因菌を突き止めるため、最新の遺伝子解析など他施設に先駆けて取り組んでいます。

(3)主要研究内容の詳細

【術中脊髄モニタリング】
齋藤教授は米国アイオワ大学に留学後、そこで学んだ体性感覚誘発電位による術中脊髄モニタリング法を関西医大に導入し、関西医科大学整形外科は日本の中でも早期から術中脊髄モニタリングを行っている施設の１つとなりました。以後、関西医大では種々の脊髄モニタリング手法を導入してきましたが、モニタリング波形の導出が技術的にハードルが高く、日本で広く普及するに到らず一部の施設でのみ術中脊髄モニタリングが行われているにすぎませんでした。1990年代になって高電圧経頭蓋電気刺激装置が開発され臨床で使用可能となってからはモニタリング波形を容易に誘発することが可能となり日本全国で術中脊髄モニタリングを施行する施設が増加しています。
高電圧経頭蓋電気刺激装置による術中モニタリング法(Tc-CMAP法)の問題点の１つは、大脳運動野を最大上刺激することは不可能であるため、モニタリング波形が誘発されない場合に脊髄に障害が生じたために誘発されないのか、刺激強度が不十分なために誘発されないのか判断できないことです。関西医大整形外科では、train刺激を繰り返すことにより振幅増大効果が得られることを見出し、2Hz前後の頻度でtrain刺激を４〜７回繰り返すことにより通常刺激により得られる誘発波形の振幅の450%に増大することを示しました。
Tc-CMAP法のもう１つの問題点は、フェード現象と呼ばれる現象で、Tc-CMAP法により術中モニタリングをしていると突然に誘発電位が記録されなくなるのを時に経験します。このフェード現象の原因は未解明のままですが、関西医大では手術時間が6時間以上経過した場合に下肢のF波検査を行い、F波の出現率や振幅が有意に低下することを見出しました。F波の出現率や振幅は脊髄の運動細胞の興奮性を反映することがしられており、脊髄運動細胞の興奮性が低下すると誘発電位が誘発されなくなるためフェード現象の原因である可能性が推察されます。

【超音波洗浄を用いたインプラント感染の細菌検出方法】
人工関節置換術や脊椎椎体固定術などはインプラントを使用しますが、重篤な術後合併症の一つにインプラント感染があります。
従来の細菌培養ではインプラントに付着するバイオフィルムなどで、細菌の同定ができないことがあります。当科で注目しているのは、感染インプラントに対する「超音波洗浄」です。超音波洗浄によりバイオフィルムを破砕し、細菌内のrRNAを摘出することができます。次に、rRNAの細菌同定には「次世代シーケンサー」を使用します。次世代シーケンサーは従来のPCR方法に比べ能率的に細菌同定をすることができます。これらの組み合わせにより、従来検出されなかった細菌感染を遺伝子レベルで検出することができる、注目すべき検査方法です。臨床研究では超音波洗浄によるRNAの検出方法の確立と、次世代シーケンサーによる細菌遺伝子同定に適した方法を確立し、臨床に応用していきます。

(4)大学院入学希望者のメッセージ

臨床と基礎は別次元ではなく、同じ土俵にあります。骨折であればなぜ血腫が必要なのか、関節リウマチであればTNF-αがなぜ治療に関係するのかなど、基礎研究を行うと臨床の現場でも一歩踏み込んだ考えができます。整形外科の基礎研究分野は多岐にわたっています。このため、整形外科講座単独では難しい研究でも、基礎講座や国内留学での共同研究などを通して、院生たちは確実に結果を残すことができます。
